Please download, print and complete application.  Then return completed PFM application, signed Rules and Regulations (only last page submit), and signed Indemnity Agreement along with Registration/Application check (10 X 10 booth fee: $60.00; 10 X 20 booth fee: $120) and all other relevant papers such as business tax license, business insurance (only if you have insurance), health department certificate, food handler’s certificate, kitchen certificate to teamtitus7@gmail.com
